Summit Overview
The Pediatric to Adult Health Care Learning Collaborative Summit will offer participants valuable insights into enhancing programs and overall state practices regarding pediatric to adult health care transition (HCT). This progress is urgently needed to support the growing number of youth with special health needs aging into adulthood.
After attending this summit, participants will be able to:
- Describe the value and need for youth and family engagement during HCT.
- Analyze evidence-based resources and apply tangible skills in the implementation of HCT processes within your own organization.
- Employ trauma-informed care and health equity principles into HCT processes.
- Gain insights from other organizations, nationally and from Minnesota, who implemented the Six Core Elements of Healthcare Transition.
This summit is hosted by Gillette Children’s in collaboration with The COPIC Medical Foundation, Got Transition, and the Minnesota Department of Health.
Target Audience
The Summit is open to those interested in advancing pediatric to adult HCT. This includes pediatric/adult clinicians from any setting, family advocates, insurers, policymakers, health system and health administrators, and more.
